Helping Today. Helping Tomorrow

Empowering Communities to Heart Health

By addressing social determinants of health and offering culturally responsive programs, screenings, education, and support, we empower individuals to prevent and manage heart conditions, leading to a better quality of life.

about the foundation

Driving Change with Passion and Purpose

Our mission is simple: to improve the early detection, diagnosis, and management of heart risk factors in people who are at high risk.

To make heart health resources accessible and useful for everyone. We aim to help communities take control of their heart health through education, screenings, and personalized support.

HGF was founded by Somaneh “Bouba.” He lived with congestive heart failure caused by his congenital heart disease for over ten years. A few years ago, he received a heart transplant and made a remarkable recovery.
