Minor Interns need four forms. The intern agreement includes a guardian co-sign section. A W9 is needed if the intern receives a stipend.

W9, IRS Tax Form
Required for interns receiving a completion stipend. Complete with your legal name, address, and taxpayer ID. HGF uses it for 1099 reporting.
