I’d like to start by thanking all of you for your generous support, buying t-shirts, donations, advice, and love. Your investment into the cause goes a long way.
Today, I want to share a few notes about what you have allowed us to accomplish.
Despite some setbacks (some expected and others not), things have been going rather well. Our primary focus has been building our team, operational readiness, and community partner network. While we designed and launched our services, we wanted first to robustly get our systems and operations to give our impact trajectory a solid foundation to benefit the maximum of people in and around our community. So, after years of planning and laying the groundwork for our community health programs, the infrastructure is ready from both a system and a staffing point of view.
Thanks to your generous support last December and grants from the AHA (from both the National and M.A. branches), BCBS MA Foundation, CVS Health, and Walmart, the above was possible. We are hopeful for future grants and more support, and we are grateful to all of you for your continued support.
HGF Team putting in work.
So far, we have managed to:
(Check out our Updates Slide Deck Here)
1. Recruit & retain +16 part-time professionals, including certified nurses and other professionals with great passion and experience in healthcare, epidemiology, public health, and service to the community.
2. Produce +12 Heart Health Articles available to the public through our Website.
3. Support the launch of the Living With heart Disease podcast, available everywhere you get your shows – Apple, Google, Amazon, Spotify,…
4. Present a 6-week Heart Health Webinar series in March and April. Close to 50 people signed up, and on average, half of them attended Live, and the Replays were made available to those who couldn’t join in at the time. As a result, we expect more people to benefit from heart health sessions. The presentation material and handouts are available on our Website. This program was made possible in partnership with the American Heart Association through the Empowered to Serve Initiative.
5. We are currently working on the video material, which we plan to share soon. In addition, we plan to develop new webinars series based on more topics and in different languages such as Haitian Creole, Cabo Verdean Creole, and Spanish. So, stay tuned and keep spreading the excellent material.
6. The Heart Health Webinar Series was an opportunity to begin our outreach for enrollment into the Healthy Hearts Communities Program. We have received a few early signees so far. But our active recruitment starts at the end of the month and throughout June and July.
Community Heart Health Education And Screening Project (June Through August)
Community Education and BP Screening – Coming This Summer!
In addition to collaborating with identified partners in the communities of Mattapan, Dorchester, Mattapan, and Lawrence (new addition), we are planning our Community Blood Pressure Screenings and Health Education Project. We will hold recuring public screening and education sessions at select locations and partner events during these sessions. Also, we will give away arm B.P. machine, B.P. monitoring card, and health and wellness educational handouts (digital and hard copies).
- Target locations include the YMCA, ABCD, and houses of faith: Churches, Mosques, Temples, or Barbershops and Salons.
- Our goal is to reach at least 200 residents of the targeted communities who are educated and screened for hypertension at no cost from June through August. And we target to get at least 50 of them enrolled for our 2HC program, which offers 3-month RPM services, nursing visits at home, and telehealth and coaching for healthy changes.
- The next goal is to continue increasing high blood pressure (or hypertension) awareness in our community to find and treat hypertension early and improve its detection and management within the adult population. This project also supports our enrollment efforts for the Healthy Hearts Communities (2HC) Program, which will entail multiple months of RPM services, nursing visits at home and via telehealth, and coaching for healthy changes.
Thanks again for joining us on this adventure. Please keep spreading the word, connecting us to good people, supporting us, encouraging, and praying.
