As we welcome 2026, the Heart of a Giant Foundation stands at an exciting crossroads. Behind us lies a year of remarkable achievements-thousands of lives touched through our community screenings, education programs, and health advocacy. Ahead of us is a year filled with even greater possibilities.
2025: A Year of Impact
Together, we made history in community heart health
879 Community Members
Reached across Greater Boston with 1,302 blood pressure screenings and culturally responsive cardiovascular education and care
19 Active Locations
Across 9 neighborhoods, from East Boston to Mattapan, Roxbury to Hyde Park, bringing heart health services directly to communities
22 Lives Protected
People experiencing hypertensive crisis identified and connected to immediate medical care
3x Retention Improvement
From 8% to 25.5%-when care is community-led and culturally responsive, people keep coming back
$75,000+ in New Grant Funding
Including support from the Cummings Foundation and Boston Scientific Foundation to sustain and grow our impact
Our Vision for 2026
This year, we are committed to scaling our impact and reaching even more families in underserved communities. Cardiovascular disease remains the leading cause of death in America, and the disparities in Black and Brown communities demand urgent action.
We believe that prevention is the most powerful medicine, and access to early screening can be life-saving. In 2026, we are focused on ambitious goals and new initiatives.
2026 Goals
Closing Health Equity Gaps
Through our new integrated three-pillar model: Community Care Connect (patient-led services), Heart Health Talent Engine (CHW training and workforce development), and Action Research Collective (community-driven research).
Creating Economic Opportunity
Building health outcomes alongside economic opportunity-training and employing Community Health Workers from the neighborhoods we serve.
Building Sustainable Systems
Generating 70% earned revenue by 2028, ensuring long-term impact beyond grant cycles.
Expanding Our Reach
Bringing our heart health services to new communities across Greater Boston.
