A familiar face changes everything

Our health workers are nurses, certified medical assistants, and community members with deep roots in the places they serve. They speak English, Spanish, Kreyòl ayisyen, Kriolu, and Vietnamese.

They know the farmers market schedule, the senior center hours, and which bus line gets you there. They are the bridge between communities and the clinical care that serves them. They make that care more reachable, more trusted, and more effective.

They are the reason people come back.
