Your gift is matched.
The Lenny Zakim Fund matching grant doubles your gift, up to our current pool, funding community health workers, screening kits, and follow-up visits.
3 · Partner
Bring the model to your site.
Health centers, faith communities, senior centers, and employers: host a screening day, embed CHWs, or co-design a referral pathway.

- Walk with us in the fall. Volunteer at a screening. Subscribe to the field notes.
- Clinicians & CHWs
- Join the joint clinical/CHW model in Mattapan, Dorchester, Roxbury, East Boston, Hyde Park, and Brockton.
- Employers & funders
- Underwrite a neighborhood, sponsor a screening day, or fund a Community Health Worker year-round.
- Press & researchers
- Site visits and data conversations welcome. We share what we measure.
Show up where it matters.
Volunteers are the backbone of Heart of a Giant, at the screening table, at the Heart Walk, in translation, and behind the scenes. Whatever your skill, there’s a role.

- Route marshals, water stations, day-of operations, and family activities.
