Between January and April 2025, the Heart of a Giant Foundation (HGF) conducted over two dozen community-based blood pressure screening and health education events across 7 Greater Boston-area neighborhoods and online: Mattapan, Dorchester, Roxbury, Jamaica Plain, East Boston, Brockton, and Bridgewater.

Program Overview

This report highlights the first quarter of our 2025 community health programming, a period of significant growth for HGF's screening and education initiatives. Our team expanded outreach to new neighborhoods while deepening engagement in communities where we've built lasting trust.

Our Q1 programming reached residents across a diverse cross-section of Greater Boston communities, each with unique health needs and cultural contexts:

  • Mattapan, Continued engagement at community centers and faith-based organizations
  • Dorchester, Expanded partnerships with local health advocates
  • Roxbury, Deepened presence at community gathering spaces
  • Jamaica Plain, New screening locations established
  • East Boston, Multilingual outreach to diverse immigrant communities
  • Brockton, Extended reach to South Shore communities
  • Bridgewater, First-time programming in this area

Key Findings

Across all screening events, our team identified significant rates of undiagnosed or unmanaged hypertension, reinforcing the critical need for accessible, community-based health services. Many participants had not had their blood pressure checked in over a year, and several received referrals for follow-up care.

Health Education

Alongside screenings, HGF delivered targeted health education on topics including understanding blood pressure numbers, dietary strategies for heart health, medication adherence, and stress management. Educational materials were provided in multiple languages to ensure accessibility.

Looking Ahead

As we move into Q2 2025, HGF plans to expand programming further, launch new partnerships, and deepen our data collection to better measure long-term health outcomes for participants. Our goal remains clear: to eliminate preventable heart disease in underserved communities through accessible care and education.
