Letter from the founder

Reflecting on 2024, Heart of a Giant Foundation stands resilient, more determined than ever to drive community health and wellness forward. This year marked my second-anniversary post-heart transplant, a journey that underscores our mission to bring quality heart health education, prevention, and care to communities most often underserved or marginalized.

Our programs grew, our partnerships deepened, and our reach extended to more individuals and families facing or at-risk of heart health challenges. None of this would be possible without our dedicated supporters, volunteers, and partners.

Programs & highlights

  • Heart Health Awareness, educational resources, workshops, and prevention campaigns.
  • Community Health Screening & Education, 802 screenings across 58 events in 11 neighborhoods; 134+ follow-up attendees.
  • On-site Health Monitoring, free hypertension and diabetes support at partner locations across Boston.
  • Living with Heart Disease Podcast, survivor stories, expert insights, and practical advice.

Hypertension screening campaign

802 screenings across 58 events; 231 high-risk individuals referred for follow-up care. Distribution: Normal 29.5% · Elevated 12.4% · HTN Stage 1 23.8% · HTN Stage 2 31.8% · HTN Crisis 2.5%.

Continuous BP monitoring & coaching

  • 68 participants monitored over 3–9 months across Mattapan, East Boston, Dorchester, and Jamaica Plain.
  • Average systolic BP decreased by 13 mmHg; diastolic by 2 mmHg.
  • Most common positive transition: Stage 2 → Stage 1 or Normal.
  • Biggest improvement: Crisis → Elevated (72 mmHg systolic drop).

Inaugural Boston Community Heart Walk

  • 200+ attendees
  • 50+ peer fundraisers
  • $10,000 raised for programs

Goals & priorities for 2025

  • Reach 3,000–5,000 individuals through screenings and education.
  • Launch between-visit support with local providers.
  • Open a 1,000–1,500 sq. ft. office for education, workshops, and staff collaboration.
  • Hire full-time CEO, Program Director, Health Manager; recruit 20+ active volunteers.
