Great news! The American Heart Association has just awarded HGF over $77,000 to implement the Healthy Hearts Communities Program (2HC) in association with their National Hypertension Control Initiative.
The AHA and HGF have the common goal to address the alarmingly high incidence of cardiovascular health conditions and complications in the US, particularly among people of color. This grant recognizes that our organization is uniquely positioned to partner with AHA to reduce hypertension among racial and ethnic groups most disproportionately impacted in the United States.
Their significant contribution to our pilot 2HC program will help raise awareness of heart disease and empower our participants to control their blood pressure and live their best lives. Moreover, this donation strengthens our existing partnership with the American Heart Association!
About the American Heart Association (AHA)
Founded in 1924, the AHA is an organization focused on transforming the lives of those affected by cardiovascular diseases. In fact, it is the largest not-for-profit funding source for cardiovascular disease research.
The National Hypertension Control Initiative centers around improving healthcare quality and empowering under-resourced communities, which are historically affected by hypertension and more recently, the COVID-19 pandemic. This initiative spares no effort in removing barriers and making strides in blood pressure control by working closely with community members, health centers, related businesses and other organizations.
About the Healthy Hearts Communities Program (2HC)
Health is a human right. At the Heart of a Giant Foundation, we have set our sights on helping everyone enjoy their lives to the fullest by empowering people to adopt heart-healthy lifestyles. Through the 2HC Program, we strive to bridge potential gaps in health education and lifestyle support to improve patient well-being and health outcomes.
The program is free to participants, so we encourage our supporters and friends to help us offset the costs by making a donation to the 2HC program. Donations are used to cover the costs of our 2HC participant needs, like biometric self-monitors and educational materials. Gifts of all sizes make a difference, and can have a long-term impact on the health outcomes of numerous individuals from various walks of life.
