Heart of a Giant Foundation
HGF has been awarded a $5,000 grant from Foley Hoag Foundation to support our Community Blood Pressure Screening and Health Education project.
Through this grant, HGF can increase blood pressure awareness in Mattapan, Roxbury, Brockton, Lawrence, and Dorchester communities, improving health outcomes for the most vulnerable residents affected by inequity.
Since 1981, Foley Hoag Foundation has awarded over $3,800,000 in grants to over 475 organizations working to reduce racial, ethnic, gender, and wealth disparities. We are one of 19 organizations awarded in the 2022 Fall grant cycle.
Our Project
Through the Community Blood Pressure Screening and Health Education Project, Heart of a Giant will offer:
- Blood pressure screening
- Training on home blood pressure monitoring
- Heart educational materials
- Tips on healthy eating and physical activities
This project supports the community outreach of the Healthy Hearts Communities (2HC) Program, which aims to empower participants to manage and prevent hypertension.
